Workers’ Compensation Claims: For any inquiries or concerns related to workers’ compensation claims, please contact us at claims@infinitihr.com.
Immediate Incident Reporting: It is crucial that all workplace injuries are reported immediately to an INFINITI HR representative, regardless of the injury's initial severity.
Injury Reporting Process: The injury reporting form must be completed without delay and sent promptly to claims@infinitihr.com.
Medical Treatment Authorization: Direct any employee needing medical attention for work-related injuries to a provider, using the authorization for treatment form provided. Additionally, provide the injured employee with the first fill form for their initial prescription. Subsequent prescriptions will be managed through a prescription card mailed after the claim is set up.
Claims Administration: An INFINITI HR representative will coordinate medical triage, assist with the claims filing process, and oversee the claims administration, including managing expectations for returning to work.
Medical Facility Location: To find an approved medical facility, visit genexservices.com/find-a-provider with the username "Next" and password "Administrators."
Refusal of Treatment: If an employee opts out of medical treatment, ensure they complete and sign the refusal of medical treatment form and keep a copy on file.
